Clinical Social Worker

Salary: $70,000-$75,000

Location: 4377 Mission st, San Francisco CA 94112 & SF satellite sites
Reports to: Director of Programs
Duties and Responsibilities:
Provide initial evaluations of client needs, obtain consent and release forms, develop customized treatment plans, implement the care plan, and coordinate discharge planning.
Provide culturally sensitive, short-term counseling to individuals and group therapy.
Provide monitored or supervised visitation for children in foster care, and their parents/guardians and ensure child safety.
Maintain an average caseload of 18-23 clients each fiscal year.
Consult and collaborate with clients, case managers, and internal/external providers to coordinate client care.
Actively follow-up with all Individualized Service Plans to assist clients to progress through their self-identified goals.
Document service sessions and maintain timely administrative and clinical records related to client care, per standards specified by APAFSS policies, clinical standards, HIPAA and utilize EMR, including Salesforce-CIRCE.
Provide crisis intervention and conflict resolution, as needed.
Provide other duties as assigned.
Minimum Qualifications:
Possession of a master’s degree from an accredited college or university in Counseling, Clinical Psychology, Sociology, Social Work, Education, or an equivalent field.
Experience working with the AAPI, BIPOC, and Latinx populations.
Experience in the field of community health programs and services.
Demonstrated competency in providing evidence-based and clinically appropriate counseling services to children, adults, families, and couples.
Productive team player who is proactive, self-motivated, creative, and organized.
Excellent communication and crisis intervention skills.
Excellent time management skills
Demonstrated knowledge and skills in community advocacy and outreach.
Strong skills in administrative support, writing, researching, and documentation.
Strong computer skills.
Excellent interpersonal and communication skills.
Demonstrated ability to show a non-judgmental attitude, and maintain client/stakeholder confidentiality, and experience working with people of various backgrounds.
Provide on-site support and triage at schools and centers.
Fluency in Cantonese.

We do this through case management and family support services at our family resource centers. By providing parents with a safe and welcoming place outside of their homes, they increase their social circles and have a forum to gain their voice.
